Stage A - application

We start the revalidation process on the third anniversary of your registration period to give you plenty of time. We will send you the revalidation form to fill in and ask you to return it in two weeks. If everything is in order, we will send your application for assessment (Stage B).

The revalidation form contains the following sections:

Information check
On the front page of the form are the details we hold about you and publish on the register. Please use the spaces provided to update incorrect information or supply anything that is missing. It is important that we can contact you easily and that the register is accurate.

Referee
If you have changed employers since you were last assessed, you will need to provide a new reference. This is subject to the same conditions as before: it must be from someone more senior than you or outside your department, organisation or company. It will speed up the process if you include this when you return the form.

Fitness to practise
Please answer all these questions, giving further information where necessary. Any false statements here will invalidate your application.

Professional experience
This form includes a new case log for you to complete. As with your initial registration, one or more assessors from your specialty choose some cases for closer scrutiny. They will ask for details of each case in the same way as before. The number of cases required though is likely to be fewer.

We also give you the opportunity to highlight a particular case which contained elements of special interest from which you learnt and which contributed to your professional development.
