The assessor's kit contains the following files:
- Form 1 - caselog
- Assessment grid (general / medical examination)
- Assessment grid (paediatrics)
- Notes for assessors
- Request for casework letter
- Outcome form
These files have been compressed into one single smaller file. To
extract (restore) the full assesor's kit, you will need to download
and install
WinRAR if
you do not already have it. This
free
programme is included as standard on many new computers.
Instuctions for download:
1. Choose your preferred format for the assessor's kit:
PDF (Adobe
Reader)
Word (Microsoft Office)
2. A download box will appear asking what you want to do with the
compressed file. Click
Save. The compressed file
will be downloaded to your computer.
3.
Right click on the compressed file and select
Extract here. The full assessor's kit will be
extracted and saved to your computer.
4. Remember to download the
general
information as well!
